Simvastatin Impairs Exercise Training Adaptations
Participants & Protocol
n=37 sedentary overweight or obese adults with ≥2 metabolic-syndrome risk factors (NCEP ATP III) completed the study: exercise only n=19, exercise + simvastatin n=18
Protocol
Randomized parallel-group trial (Exercise, Statins, and the Metabolic Syndrome; NCT01700530) in sedentary overweight or obese adults with at least 2 metabolic syndrome risk factors (from the abstract).
Randomised to 12 weeks of aerobic exercise training alone or with simvastatin 40 mg/day; primary outcomes cardiorespiratory fitness and vastus lateralis citrate synthase activity
Key Results
"Exercise only" group: cardiorespiratory fitness rose 10% (p < 0.05)
"Exercise + simvastatin" group: only a 1.5% increase (p < 0.005 for the group × time interaction)
Muscle citrate synthase activity (a marker of mitochondrial content) rose 13% without statin (p < 0.05) but fell 4.5% with statin (p < 0.05 for the interaction)
Conclusion: simvastatin blunted both the functional (CRF) and molecular (mitochondrial) adaptation to training
Practical Application (not medical advice)
Mikus et al. 2013 (RCT, n=37 sedentary adults with metabolic-syndrome risk): 12 weeks of aerobic training raised fitness 10% alone but only 1.5% with simvastatin 40 mg/day, and muscle citrate synthase rose 13% without the statin but fell 4.5% with it. In this population the statin blunted both the fitness and the mitochondrial response to training.
Discuss with a physician if there is an unexplained fitness plateau while on statin therapy
Clinical relevance for trained athletes is not established (sample = sedentary, metabolic syndrome)
Limitations
Sample = sedentary with metabolic syndrome, NOT trained masters athletes
Simvastatin only (lipophilic); hydrophilic statins (pravastatin, rosuvastatin) may differ
n=37 (19/18), 12 weeks

Tier 1 -> Tier 0 (sedentary); "age ~25-59" removed (not in the abstract). Group sizes (19/18), NCEP ATP III criteria and p-values added from the abstract; lead Practical bullet added with the verified numbers. Level B+ kept (parallel RCT, n=37).
2026-09-28: added a Protocol heading and design bullet from the abstract (Europe PMC PMID 23583255).
